Mortimer Physiotherapy Practice is committed to protecting your privacy. Any information you provide us with will be used only in accordance with this privacy notice.

Mortimer physiotherapy is registered with the Data Protection Agency and this notice explains how and why we collect and control data to comply with the Data Protection Act and General Data Protection Regulation (GDPR)

WHAT INFORMATION DO WE HOLD AND WHY?

We hold personal contact and medical information on patients

Contact Information includes

-Full name and date of birth

-Contact address

-Telephone numbers and email address

Medical information will include details provided by GP and/or referring consultant, clinical information collected directly from the patient and clinical notes taken by the treating therapist with our consent form and at subsequent consultations.

We do not collect or store any bank details at any time

The information we hold is exclusively for the purpose of managing the treatment of our patients and communicating with them and other appropriate healthcare professionals to provide safe and effective healthcare.

Communication information is used to arrange appointments and respond to any patient enquiry.

Medical information is held to document and track treatments as part of your confidential and secure medical records. This may include:

Referral documentation supplied by your GP or consultant

Relevant medical history, documented conditions and medication

Details of each treatment

Discharge and clinic reports

HOW LONG DO WE HOLD INFORMATION?

We hold records for a minimum of seven years in accordance with legislated best practice. After this time any physical records are securely shredded unless relevant to ongoing treatment.

WHO MIGHT WE SHARE INFORMATION WITH?

Information will only be shared with other medical and health care professionals involved in the provision of care- generally your GP, consultant or private health care provider. A report will be sent to the referrer (usually GP or Consultant) on discharge.

Mortimer Physiotherapy will not provide any data to third parties unless requested to by law. We will never sell data for marketing or analysis purposes.

HOW DO WE ENSURE THE SECURITY OF INFORMATION?

All employees of Mortimer Physiotherapy Practice are subject to confidentiality agreements and are trained to ensure compliance with our security policies.

Paper based information is kept securely within the clinic and all electronic data is held on systems requiring log in and is password protected. Any information transferred outside the clinic is either by encrypted email systems or secure document pouches.

WHAT ARE MY RIGHTS TO THE INFROMATION HELD?

The individual’s rights with regard to data have been increased under the new GDPR.

You now have:

-The right to be informed-you have the right to understand the type of information held and how it is used-this policy provides you with this.

-The right to access-you have the right to see or have copies of your personal information. If you would like a copy of the information we hold on you please write to Mortimer Physiotherapy Practice, 22 The Avenue, Mortimer, Berks, RG7 3QY, or email info@mortimerphysio.co.uk a small fee may be payable.

-The right to request that your personal information be rectified-it is important that the information we hold on you is correct and updated, for example if your contact details change. Any clinical information will remain valid as they were the opinion at the time of being recorded.

-The right to Erasure-GDPR generally allows an individual to have their information deleted unless there are compelling reasons not to do so. Health care information has a general exemption as it may be vital to the patient’s healthcare.

-The right to restrict processing- we do not carry out processing on any data we hold.

-The right to data portability-we do not process personal information so the right to data portability is not applicable. However you still have the right to request this.
